Family & Children’s Medicaid

How to apply for Medicaid or NC Health Choice

There are three ways to apply (one application works for Medicaid and Health Choice).

  1. Apply online at ePASS.
  2. Apply in person at the Stanly County Department of Social Services:
    Stanly County Commons, 1000 North First Street, Suite 2, Albemarle, during normal hours of operation. Normal hours are Monday – Friday, 8:30am until 5:00pm.
  3. Fill out a paper application and mail to or drop it off at your county DSS office.

Items that you may need to complete your application:

  • Medicare cards
  • Social Security cards
  • All life insurance policies
  • All health insurance policies and/or cards and the premium paid
  • Recent bank statement (including checking and savings accounts)
  • All information concerning assets such as stocks, bonds, certificates of deposits, IRA’s or other retirement accounts
  • Pre-paid burial contracts with a funeral home
  • Medical bills
  • If working, proof of income
  • If self-employed, most recent tax return
  • Proof of unearned income (retirement, etc.) except Social Security Income
    All these items are needed for both spouses.

Program Descriptions


Family & Children’s Medicaid or NC Health Choice

Medicaid for Pregnant Women (MPW)

Medical assistance is available for pregnant women who fall below the income guidelines. The coverage is for pregnancy related procedures only. Once determined eligible for MPW, the coverage continues through the postpartum period regardless of income or other changes.

Medicaid for Families (MAF)

Medical assistance for low-income families with children under 19. To qualify the family must meet income guidelines and resource limits (bank accounts, vehicles, and other assets). This category includes 19 and 20-year-old individuals that may or may not have a child. Some families may have a deductible to meet before Medicaid coverage begins.

Medicaid for Infants and Children (MIC)

Medical assistance for children or individuals under the age of 19. There are three different income categories based on the age of the individual. The income limits are highest for very young children. There are no reserve limits or deductibles for this program.

North Carolina Health Choice (NCHC)

Medical insurance administered by Blue Cross Blue Shield. This program is for individuals under age 19 who’s income is too high for Medicaid and who do not have private insurance. Depending on the family’s income there may be co-payments for prescriptions, doctor visits, and emergency room visits. Some families may need to pay an enrollment fee of $100 maximum before benefits are issued. This enrollment fee covers the child/children for one year.
